What's Killing My Juvie Hawthorne Tree?

A couple of weeks ago, I noticed that my Toba Hawthorne was struggling to leaf out. The leaves aren't growing to maturity, the branches look twiggy, and it looks sad relative to the canopy it had last year, despite a prolific flowering season.

Now, a few weeks later, I investigated the bark more closely and noticed holes, peeling bark, and what appears to be evidence of a borer or insect.

Any ideas on what's killing my tree and how to fix it? Thanks as always.
